Everything under the FHIR base needs a token. The cohort surfaces do not: they are rig operating surfaces holding synthetic data, and a human opening this in a browser is not holding a token. Their paths carry _fake so that any Arca code reaching for one is greppable -- nothing on a real source answers here.
